the Il-86 has had one fatal crash (which did not involve passenger fatalities), which looks to have been caused by an issue with the aircraft itself, though I've not been able to find much detail on the incident in English.
It was determined to be a pilot error.
There were three more non-fatal hull losses - once when a 737 fell on it, and two large pilot errors.

Some problems had been because the M-16 had been designed for indoor use as it seems,

Same problem as the F-35 JSF, eh?

If you already have a dirty rifle and did not oil it and clean it properly, you can make things still much worse by firing automatic until your magazine is empty.

I've had that happen to me with a semi auto 12 gauge. Nasty surprise!!! Even if there were only 3 rounds left. Never assume everyone else treats firearms as carefully as you do.

Was it a Franchi SPAS? Used to happen a lot with the SPAS-15, it's very ammo sensitive. Anyway, there's a darn good reason I want to inspect and clean personally any firearm I put my twitchy little hands on.

I've had that happen with Nerf rifles. There is almost nothing worse than pulling the trigger, hearing the loud clack of the pneumatic mechanism fire -- and have nothing happen in the middle of a skirmish.

Alllllllllllllllllllthoughhhhhhhhhhhhhhhh . . .

The flip-side of the coin is that it's immensely satisfying watching your target hear your rifle go off and spontaneously duck for cover, avoiding the non-existent projectile. Repeatedly.

Couldn't the ammo go of in the barrell ripping it apart? And depending on the rifle and its handle your hand?
Or am I just making stuff up I saw in a Hollywood movie a decade ago?

There is... Having it happen with the real deal.

That's when you unsheathe your quite impressive and totally not compensating Claymore and charge the enemy with fire in your veins, and bloodlust in your heart.

or whack it a few times...either way works

Thats Hollywood physics based on the behavior of old cannons of doubtful quality. The worst that can happen to you if a bullet in a rifle gets stuck (which can happen) is that all the hot exhaust gases go out the wrong way... your way.

A properly produced barrel is usually not exploding, unless you use a propellant it was not certified for.

I found out that the owner had used some old water-damaged paper cartridges that bent one of the stopper pins. I had a sore shoulder for a day or two.

My backup has calibre 20mm and drives 70 km/h without cheating (130 km/h with cheating)...

You know, that thing goes for three, maybe four hours without refueling. A true soldier goes for seven hours and then asks his buddy if he needs a break or they can continue throughout the night.
