Those weren't exactly 'failures' as the missions weren't officially compromised. STS-51-F failed to reach its intended orbit, ergo (happily non-catastrophic) failure.What about STS-27 and STS-114? Near-fatal tile damage on the former and some extruded filler that required repair on the latter.
