You are set up wrong for this flight. For an Earth - Moon trip, Surface Launch program should be set to Lunar Off Plane, not Course. You will need to know about when you will get to the Moon - Surface Launch needs that to figure out the correct plane.

Surface Launch works with Course when flying interplanetary trips. In that case, the Source (in target intercept) would be the planet you are starting from - not your vessel. Then Surface Launch will provide info on time and heading to get you into the ejection plane, and Orbit Eject is used to perform the transfer burn.

After you leave the "Source's" SOI< change the Source to your vessel to make MCC's.
